Our staff will be happy to help you decide which type of appointment is best for you.

Please plan on arriving about fifteen minutes ahead of the scheduled time for your first visit to complete the necessary paper work. New patient forms are also available to print from our website. Please remember to bring these items for your first visit:

    * Medications you are currently taking

    * Your eyeglasses

    * Contact lenses

    * Insurance cards

    * Referral authorization if required by insurance

    * Driver's license/photo ID or utility bill or other correspondence showing current address if the photo ID does not show current address

There will be a $50 fee after three (3) missed appointments without notice. A 24 hour notice is required for all appointment cancellations.

After you have completed the information form and before you see the doctor, one of our skilled technical staff members will compile your medical history and perform any necessary tests. The doctor will examine you and discuss your condition.
